Online Registration

Beginning with the 2019-2020 school year, school registration will be online.  Simply log in to Skyward Family Access and click on "2019-2020 Online Registration" on the left side of the screen.  You must complete each step in order to successfully register your child.  Additionally, online registration must be complete, and any required documents must be turned in before your child can attend school.  If you have any questions, please call the main office.  Click here to access Skyward Family Access.  

Online Registration for the 2019-2020 school year will begin on July 22, 2019.

State of Illinois required documents for the 2019-2020 school year:

9th Grade: 
Current physical within one year of first day of school (must be a complete physical, NOT a sports physical), Immunizations as per requirements from previous grade levels, plus proof of having received at least one Tdap booster, a second varicella vaccine, and proof of one Meningococcal vaccine.

12th Grade: 
Proof of having received 2 doses of meningococcal vaccine w/ the 2nd after age 16.  
All 12th graders will be required to have a meningococcal vaccine (meningitis) on/after the age of 16.  The Student Health Center will have the meningitis vaccine available from now until the end of the school year, if you would like your student to receive it while at school.   Linked below is the paperwork for your student to receive the vaccination at Pontiac Township High School District 90.  We can bill your insurance; however, it is your responsibility to make sure your insurance will cover the cost.  If you do not have insurance, the cost is $20.  We can also bill Medicaid.  Proof of this immunization will be required in the Student Health Center before students can register in July.   If your student has already received this immunization, please forward a copy to the Pontiac Township High School District 90 Student Health Center.  We are pleased to offer this service to the students at Pontiac Township High School District 90.  If you have any questions, please call the Student Health Center (815) 844-6113, ext. 255.
